Covid Proof of Vaccination

To attend WIBC in Seattle, vaccinations are required. Please upload your proof of vaccination (a picture of your card).

Note: You must register for WIBC before you can upload your card. If you are vaccinated, but do not yet have your card, you may return later and upload it by the deadline (November 18, 2021).

Upload your Vaccination Card or 11/18/2021 Negative Test Result

* If you Request an Exemption, you will be required to upload your Negative Covid test result taken on November 18, 2021 (not earlier).

COVID-19 Tests/Results: WIBC will accept any COVID-19 test that is administered (no earlier than 11/18) and documented by a healthcare professional, pharmacy or school clinic. Documentation (PDF or JPG) must be official and include the patient's name, date of test, and location. See Example Take home tests will not be accepted.

We understand this process is an inconvenience, but it is necessary to maintain a safe environment for all involved.
